Paper Enrollment ID (pEID)
This is your standard issue enrollment ID, it is free and you will be given one at the time of enrollment. Your pEID is sufficient for all member benefits.

Replacement pEID
All pEID replacements are no cost to you, please email the enrollment office at midwestlosthistory@gmail.com

Hard Photo Enrollment ID (hEID)
A hard photo hEID can be ordered HERE, if you'd like a photo attached please come to the culture center to have your photo taken or provide a professional face forward, close range headshot. A Hard photo ID is not necessary for members benefits, but can suffice as a second form of identification in some circumstances.

Replacement hEID
hEID replacement including lost, damaged, stolen, or update requested is standard price. hEID will only be updated upon request.
All members are assigned an enrollment ID number and a paper enrollment ID card. Plastic photo ID can be purchased for a fee, but are not necessary.
